Do you need to be hospitalised? A nurse coordinator from the Help network is there to guide you through the process and act as a link between you and the hospital – providing personalised support and simplifying every step of your journey.
Nurse Coordination: your partner for a successful return home
As soon as your discharge from hospital is being considered, you can request a meeting with a nurse coordinator from the Help network, who will become your main point of contact. They will ensure that every step of your return home is carefully planned, coordinating communication between all involved professionals (doctors, specialists, care providers) so that everything is ready for your arrival.
Steps of the support process
Initial contact
Ask the hospital team to arrange a meeting with our nurse coordinator.
Needs assessment
Together with you and your loved ones – and in collaboration with the hospital team – the nurse coordinator assesses your care and support needs to ensure a safe return home.
Organisation of your return
Our team then coordinates all the services required for your home care, ensuring everything is in place before you arrive.
Follow-up care at home
You receive personalised care based on the established plan, with ongoing coordination between your general practitioner and the hospital team.
